The goal of the Ways2Wellness CONNECT Program is to provide tools to positively impact the health and well-being of residents of the Coos, Carroll, and Northern Grafton Counties of NH.
Everyone’s life and circumstances are different, to achieve this goal the North Country Health Consortium (NCHC) has developed a couple different pathways to success. These paths are the Ways2Wellness website and the Ways2Wellness CONNECT Program.
The Ways2Wellness webpages are for those of us looking for ways to make small changes to our lifestyles that can impact our health in big ways. The Ways2Wellness CONNECT Program webpages focus on the residents of Coos, Carroll, and Northern Grafton Counties who are 18 and over and dealing with the difficulties of Chronic Disease. This program is based on the idea that “health starts at home” and provides a trained Community Health Worker (CHW) to help clients reach their goals.
